This page confirms that your hearing request has been submitted.

 

Information Summary

 

The number displayed on the screen is your Hearing Request Submission ID. If you call the Benefit Center to find out the status of your hearing request, you will need this number. You should write this number down. This number is not the same as your Client Identification number.

 

Printing Your Application

 

If you would like to print or save your hearing request, select the Print Information button to see a copy of your hearing request. You should print this out and keep it for your records. Your hearing request will not be available for viewing on you MyAccount, so if you want to view it late, you should print or save it now. If you don't have a printer, you can save the application to your computer.

 

To view the printable version, you will need Adobe Acrobat Reader. Most computers have this program, which will start automatically when you click the Print My Information button. If your computer doesn't have this program, you can install it for free by clicking the on the link that says "Adobe Acrobat Reader." Once you click this link, follow the steps it gives you to download and install the software.

 

This hearing request has your private, personal information on it. If you're printing in a public place, such as a library, keep track of where it will be printed.
