This page confirms that your application has been sent to a DSS office.

Keep Track of Your Application

The number displayed on the screen is your ConneCT Application tracking number. If you call or visit a DSS office to find out the status of your application, you will need this number. You should write this number down. This number is not the same as your Client Identification number. A Client ID number will be assigned after your application is processed by a worker.

Printing Your Application

If you would like to print or save your application, select the Print PDF button to see a copy of your application. You should print this out and keep it for your records. The print version includes a one-page summary of the next steps you need to take. If you don't have a printer, you can save the application to your computer.

To view the printable version, you will need Adobe Acrobat Reader. Most computers have this program, which will start automatically when you click the Print My Application button. If your computer doesn't have this program, you can install it for free by clicking the on the link that says "Adobe Acrobat Reader." Once you click this link, follow the steps it gives you to download and install the software.

This application has your private, personal information on it and could be as much as 20 pages long.  If you're printing in a public place, such as a library, keep track of where it will be printed.

Print your Voter Registration Form

This section will only display if you selected the yes radio button to register to vote on the previous page. Click the Print Voter Registration button to print the form. Once you print and sign the voter registration application, you can:

•   Mail it directly to the registrar of voters in your Town Hall or;

•   Mail to the Department of Social Services.
