Medical
Expenses
If
someone in the household has ongoing medical bills or has unpaid medical
bills, select the checkbox next to their name. If their bills will be
paid by an insurance company or government program, like Medicare or Medicaid,
do not select their name.
If
someone has out-of-pocket costs not covered by insurance or a government
program, select the checkbox for anyone who has an expense.
Some
of these expenses include but are not limited to:
- Medical
and dental care, including psychotherapy and rehabilitation services,
provided by a state licensed practitioner or other qualified health
professionals.
- Hospitalization
or nursing care.
- Prescription
drugs, including the cost of postage for mail-order prescription drugs.
- Over-the-counter
medication (including insulin) and other health-related supplies (bandages,
sterile gauze, incontinence pads, etc.) when recommended by a licensed
health professional.
- Premiums
you pay for health and hospitalization insurance.
- Dentures,
hearing aids and prosthetics including the cost of securing and maintaining
a seeing eye or hearing dog or other assistance animal.
- Purchase
and maintenance costs of any animal specifically trained to serve
the needs of disabled people, including the cost of food and veterinarian
care.
- Eye
glasses and contact lenses prescribed by an eye doctor.
- Reasonable
cost of transportation and lodging to obtain medical care.
- Costs
for an attendant, homemaker, home health aide, or housekeeper necessary
due to age, infirmity or illness.
- Payments
made on a loan's principal if the loan was used to pay a one-time
medical expense.
- Lifeline
or Medic Alert devices used to contact medical help in emergencies,
if ordered by a licensed practitioner or other qualified health professional.
