Job Loss or Reduced Hours

If you answered that someone in the household lost a job or their hours have been reduced, then answer these questions.

 

Enter which job changed.

 

Select the type of change that the person’s job experienced.

 

Enter the date the job ended in the following format: MM/DD/YYYY. Be sure to use slashes (/).

 

Enter the date of last pay in the following format: MM/DD/YYYY. Be sure to use slashes (/).

 

Enter the date the hours were reduced in the following format: MM/DD/YYYY. Be sure to use slashes (/).

 

Enter the reason hours were reduced.

 

If the person has another job that was lost or the hours were reduced , select Yes. Enter information about the additional job loss and reduced hours the same as you did for the first job.
